Pacesetters Home Health is a Medicare-certified home health agency providing multi-specialty care. With a team consisting of professionals from six disciplines, we are able to offer clinicians with various realms of expertise who participate in each patient’s individualized plan of care.
We take responsibility for coordinating home care services with other providers such as home pharmacies and medical equipment companies. When patients have orders from multiple physicians, we collect all relevant information and incorporate it into one comprehensive home health plan of care.
